    Description: Tailored to district needs, this optional form can be used to collect student registration information from schools. All NIMAS-eligible student information must be added to the NIMAS/Florida database by the District Digital Rights Manager (DRM) prior to ordering texts in specialized formats.

  • Sample Physicians Certification Form
    Description: Tailored to district needs, this optional form can be used to collect information from medical doctors when certifying students with reading disabilities. This form also includes four questions and answers directed toward medical doctors.

  • Who Can Access NIMAS-Derived Files
    This is a one-page graphic illustrating Who Can Access NIMAS-Derived Files? The chart identifies "competent authorities" and "certifying professionals" by each of the NIMAS-eligible disability areas.
